APPLICATION FORM

The application can be handwritten in a printed format, but preferably

typed.

1. Grade transcripts from high school, college, and/or technical schools
must accompany the application. Accumulative GPA of 2.8 is required
from the Allied Health Program to be considered for the Ultrasound
Program. The schools can send these or the student can obtain an
official copy from the school in a sealed envelope only to be opened
by the program director.

2. Three letters of reference from professional associates must be
submitted.

3. ALL applications, transcripts, and reference letters must be
received by no later than July 1, 2003 to be considered for the
program.
